Fracture and dissociation through the midfoot may progress to a dorsal dislocation of the metatarsals. Thereafter, patients are transitioned to a short-leg walking cast or boot and progressive weight bearing is begun. Most significant have been the dramatic changes in the clinical practice of anesthesiology and the scope of anesthesia services (see Chapter 1). In our opinion, these two entities are separate, and with careful examination plantar fasciitis may be isolated and effectively treated with endoscopic plantar fascia release.
